NutriPacks Website - If your child can benefit from nutritious food, complete the NutriPacks registration form. Forms are available from Pupil Services office and on the NutriPacks website http://www.cdsdnutripacks.com.  

Send the completed form to school with your child or email it to your schools site facilitator. Site facilitator names/schools are listed on the website under "How to Enroll".
