Kindergarten: Height, Weight, Near Vision, Far Vision

1st Grade: Height, Weight, Near Vision, Far Vision, Color Vision and + Lens & Hearing

2nd  Grade: Height, Weight, Near Vision, Far Vision, Depth Perception & Hearing

3rd  Grade: Height, Weight, Near Vision, Far Vision & Hearing

4th Grade: Height, Weight, Near Vision & Far Vision

5th Grade: Height, Weight, Near Vision & Far Vision

 **Only if any abnormal results are found, parents/guardians will be             contacted by a formal letter.

     However, if you have a concern regarding your child's screening               results, PLEASE do not hesitate to contact your child's nurse!**
