- What is Healthy Roster? Healthy Roster is an online tool used by the CDSD Athletic Trainers.
- It helps them keep track of:
- Sports Paperwork
- Medical Records
- Injuries
- Doctor’s Notes
- It is safe, secure and private.
- It helps them keep track of:
- Why are we using Healthy Roster? Healthy Roster makes sports clearance easier and faster for families and the school.
- Parents and Guardians fill out CDSD & PIAA forms online
- Forms can be checked and approved more quickly
- Doctor’s Notes can be easily uploaded to the portal by the parent/guardian

NEW Athlete Instructions
NEW Athlete Instructions
For any new athletes in grade 7 through 12 who have never had a Healthy Roster profile.
- Step 1: Create a New Athlete Profile
- Complete the NEW Athlete Profile Survey:

- You will get a confirmation email from the East Athletic Trainers
- The email will come from a cdschools.org address
- This email lets you know your student-athletes profile was created
- Complete the NEW Athlete Profile Survey:
- Step 2A: Create Your Healthy Roster Account
- An invitation email will be sent to the email address from the survey
- This will come from:
- Open the email with the subject:
- “Join Central Dauphin School District on Healthy Roster.”
- Click on the link in the email that says: “Click here to sign up via the website instead”
- When asked - Enter the 6 Character Code from the invite email
- Each athlete has a different code
- Follow the steps on the screen to finish setting up your account
- Step 2B: I Already Have a Healthy Roster Account
If you already have an account and are adding a new athlete- Use the same email address as your existing account when filling out the Google Survey
- Your new athlete will be added automatically to your account
- In the app, swipe left or right to switch between athletes
- Step 3: Complete Digital Documents
You must complete all “Missing Documents” for your athlete- Select the athlete that you would like to complete
- In the app - swipe left or right to switch athletes
- Select the “Documents” tab
- “Missing Documents” will be highlighted in RED
- Complete all required “Missing Documents”
- PIAA Sections 1, 2-4, 5, and CDSD Sports Supplemental Form
- How to Fill Out Documents
- Website: Select each “Missing Document” then “Fill Online”
- App: Select each “Missing Document” click “NEXT” then “Complete Form”
- Note: For PIAA Sections 2–4 and 5:
- – The Signer Name must match the Signing Role
- – Forms digitally signed by the student name instead of the parent/guardian will be rejected
- PIAA Section 6 (PIAA Physical Doctor Form)
- All physicals for the 2026-27 school year MUST be dated: May 1, 2026 or later
- Must be completed, signed and dated by an Authorized Medical Examiner (AME) on PIAA Section 6
- Uploading completed PIAA Section 6 to Healthy Roster app
- – Select “PIAA CIPPE Section 6”
- – Click “NEXT”
- – Choose “Scan Document with Camera”
- – Click “SAVE” to Upload
- Documents “Need Review” – Final Step Before You are Cleared
- Completing forms does not mean they are approved right away.
- Athletic Trainers must review and approve all forms.
- After you submit forms, check your account to make sure none were rejected.
- If a form is rejected, it will show again under “Missing Documents.”
- If a form is rejected, you will get an email from
no-reply@notifications.healthyroster.com explaining why it was rejected and what needs to be fixed.
