CD East Sports Physicals

School Sponsored Pre-Participation Athletic Physicals
All physicals for the 2026-27 school year MUST be dated May 1, 2026 or later

  • CDSD will offer free sports physicals for students provided by Penn State Health.
  • Forms completed during the school sponsored exam will be returned to the student-athlete for the parent/guardian to upload to Healthy Roster.
  • Dates & Location
    • Tuesday, June 2, 2026: 3-5:45pm
    • Thursday, July 30, 2026: 3-5:45pm
    • Location: CD East High School Main Gym
  • Who Can Attend
    • Students in grades 7–12 for the 2026–2027 school year
    • Students trying out for a CDSD school-sponsored sport
    • Covers Fall, Winter, and Spring sports for the 2026–2027 school year
  • What You Need to Do
    • Select the Sign-Up Link below
    • Sign up one student per time slot
    • Enter the student’s name and parent/guardian email
    • Bring PIAA Section 5, filled out and signed
      • The completed form can be printed from Healthy Roster
      • Explain all YES answers on PIAA Section 5
  • Important Reminders
    • No sign-ups after June 1 for June 2
    • No sign-ups after July 29 for July 30
    • Walk-ins may be allowed, but are not guaranteed
