IMMUNIZATION REQUIREMENTS FOR THE SCHOOL ATTENCENCE!
Pennsylvania state now requires that all doses of immunizations be completed within the first 5 days of school to prevent exclusion. Students need to have 2 doses of the Varicella (chicken pox) and MMR (measles, mumps and rubella) vaccines given at or after the age of 12 months, 4 doses of the DPT and Polio vaccinations with the last dose given at or after age 4 years old, and 3 doses of Hepatitis B vaccine with the last dose given at or after age 6 months.
REQUIRED SCHOOL EXAMS
As a reminder:
Physical examinations are required for all new entrants (kindergarten or 1st grade).
Dental examinations are required in for all new entrants (kindergarten or 1st grade) and for students in 3rd grade.
Exams may be done up to 12 months prior to the first day of the required school year. If you plan on having your child’s state mandated exams done privately, and need a new form, please call so we can provide you with one or check the link to download a form to your computer.
No medication is administered by School Personnel unless specific written instructions are received from a physician. This applies to prescription and non-prescription drugs and topical medications as well (the only exceptions are cough drops with a parent permission note, and approved over-the-counter medications that have signed permission from a parent on the current annual health survey).
