    IMMUNIZATION REQUIREMENTS FOR SCHOOL ATTENCENCE!
    Pennsylvania State now requires that all doses of immunizations to be completed within the first 5 days of school to prevent exclusion. Students are required to have 2 doses of the Varicella (chicken pox) and MMR (measles, mumps, and rubella) vaccine given at or after the age of 12 months, 4 doses of DPT and Polio vaccines with the last dose given at or after age 4 years, and 3 doses of Hepatitis B vaccine with the last dose given at or after age 6 months.

    As a reminder:
    Physical examinations are required for all new entrants (kindergarten or 1st grade).  
    Dental examinations are required for all new entrants (kindergarten or 1st grade) and for students in 3rd grade.
    Exams may be done up to 12 months prior to the first day of the required school year.  If you plan on having your child’s state mandated exams done privately, and need a new form, please call so we can provide you with one, or check the link to download the form to your computer.
     
                                       MEDICATION POLICY
    No medication is administered by School Personnel unless specific written instructions are received from a physician.  This applies to prescription and non-prescription drugs and topical medications as well (the only exception are cough drops with a parent permission noted and approved over-the-counter medications that have signed parent permission on the current annual health survey).
